The Home Health Care CAHPS® Survey (HHCAHPS) Mode Experiment. Use. The reporting of quality data by HHAs is mandated by Section 1895(b)(3)(B)(v)(II) of the Social Security Act (âthe Actâ). This statute requires that âeach home health agency shall submit to the Secretary such data that the Secretary determines are appropriate for the measurement of health care quality. Such data shall be submitted in a form and manner, and at a time, specified by the Secretary for purposes of this clause.â HHCAHPS data are mandated in the Medicare regulations at 42 CFR 484.250(a), which requires HHAs to submit HHCAHPS data to meet the quality reporting requirements of section 1895(b)(3)(B)(v) of the Act.
This collection of information is necessary to be able to test updates to the HHCAHPS survey and administration protocols. CMS proposes to conduct a mode experiment with the main goal of testing the effects of a web-based mode on response rates and scores as an addition to the three currently approved modes (OMB Control Number. 0938-1370). The addition of a web mode will give HHAs an alternative or an addition to the use of mail and telephone modes. CMS is also interested in testing a revised, shorter version of the HHCAHPS survey, based on feedback from patients and stakeholders.

The Home Health Care CAHPS® Survey (HHCAHPS) Mode Experiment. Use. The reporting of quality data by HHAs is mandated by Section 1895(b)(3)(B)(v)(II) of the Social Security Act (âthe Actâ).
This statute requires that âeach home health agency shall submit to the Secretary such data that the Secretary determines are appropriate for the measurement of health care quality. Such data shall be submitted in a form and manner, and at a time, specified by the Secretary for purposes of this clause.â HHCAHPS data are mandated in the Medicare regulations at 42 CFR 484.250(a), which requires HHAs to submit HHCAHPS data to meet the quality reporting requirements of section 1895(b)(3)(B)(v) of the Act. This collection of information is necessary to be able to test updates to the HHCAHPS survey and administration protocols.
CMS proposes to conduct a mode experiment with the main goal of testing the effects of a web-based mode on response rates and scores as an addition to the three currently approved modes (OMB Control Number. 0938-1370). The addition of a web mode will give HHAs an alternative or an addition to the use of mail and telephone modes.
CMS is also interested in testing a revised, shorter version of the HHCAHPS survey, based on feedback from patients and stakeholders. The data collected from the HHCAHPS Survey mode experiment will be used for the following purposes. Test the shortened survey instrument, including several new items.
Compare survey responses across the four proposed modes to determine if adjustments are needed to ensure that data collection mode does not influence results. And Determine if and by how much patient characteristics affect the patients' rating of the care they receive Start Printed Page 42843and adjust results based on those factors. The mode experiment is designed to examine the effects of the shortened survey on response rates and scores and to provide precise adjustment estimates for survey items and composites on the shortened survey instrument.

Effective community engagement includes cross-sector representation, participation of vulnerable populations, and education on the economics of local health care services.
Community members must understand that health systems are not âpublic utilitiesâ but resources requiring local utilization and financial support. Effective community engagement seeks to identify and reflect local concerns, values, and priorities. It should also explore why residents bypass local services to seek care outside of the community.
Communities will need tools, technical assistance, and resources to support their community engagement processes.Cornerstone 3. Local health planningCommunity engagement and local health planning are closely aligned. Local health planning processes are not the large-scale programs created under the National Health Planning and Resource Development Act of 1974.
Rather, they are local efforts that can leverage the community health needs assessments (CHNAs) required of tax-exempt hospitals or the Mobilizing for Action through Planning and Partnerships (MAPP) process, used by public health agencies for voluntary accreditation. These processes offer a framework to conduct community health planning and engagement focused on health rather than health services.Collaboration between hospitals and local health departments (LDHs) would result in more comprehensive community health assessments. Maryland, New York, North Carolina, and Ohio encourage collaboration between hospitals and LHDs and/or the alignment of their assessment cycles.
New York requires hospitals and LHDs to collaborate on CHNAs, prioritize community issues, and jointly implement initiatives to address health priorities. To maximize their effectiveness, these assessments and planning processes should reflect the health system and health improvement needs of the community.Cornerstone 4. Rural health systems often compete in âmedical arms racesâ for specialty and diagnostic services, resulting in duplication and inefficient resource use. In contrast, regionalization involves ârightsizingâ health systems by organizing delivery of essential services locally and high-cost services regionally. The loss of rural obstetrical services is an opportunity to regionalize care by providing pre/postnatal services locally, performing deliveries at designated regional hospitals, and offering transportation to ensure access to regional services.Effective planning and regionalization require local and state-level input on the distribution of rural populations, needs, and services.
States can play an important role in encouraging regional health planning. Texas, for example, funded Regional Health Partnerships (RHPs) under a Medicaid 1115 waiver. RHPs, which include hospitals and LHDs.
RHPs must create plans to improve regional access, quality, cost-effectiveness and collaboration. Florida, as another example, established local health councils which are non-profit agencies that conduct regional health planning and implementation activities.Regional health planning can also support coordinated preparedness and response to local and global events. Minnesota, for example, established eight Health Care Coalitions that collaborate inter-regionally for planning and response purposes.
State Offices of Rural Health and other stakeholders can facilitate regional planning by convening health care, public health, and social service partners.With Crisis Comes OpportunityRural America has an exceptional history of resilience, innovation, and collaboration. Recovery from asthma treatment requires new strategies to rebuild the crumbling rural health foundation. The four cornerstones â payment and delivery system reform, community engagement, local health planning, and regionalization â can provide the base for strong and vibrant health systems serving rural America.Tools and resources are needed to support rural communities in taking responsibility for their health systems.
Government and philanthropic organizations can be an important source of funding for development of these resources. A greater percentage of postmenopausal women had frequent trouble falling asleep, staying asleep, and not waking well rested compared with premenopausal women. Data for this analysis came from the Sample Adult core and cancer supplement sections of the 2015 NHIS. Estimates on sleep duration and quality in this report are nationally representative of the civilian, noninstitutionalized nonpregnant female population aged 40â59 living in households across the United States.
The sample design is described in more detail elsewhere (7). Point estimates and their estimated variances were calculated using SUDAAN software (8) to account for the complex sample design of NHIS. Linear and quadratic trend tests of the estimated proportions across menopausal status were tested in SUDAAN via PROC DESCRIPT using the POLY option.
Differences between percentages were evaluated using two-sided significance tests at the 0.05 level. For many of these tests, self-collection is under review or online pharmacy ventolin is being performed as a clinical trial.As of July 5, 2021, the Lucira âCheck Itâ asthma treatment Test Kit is the only self-test kit approved by Health Canada. It is used as an over-the-counter self-test in people aged 14 and older.âCheck Itâ is a nucleic acid amplification self-test that works with self-collected nasal samples.
Results are provided in 30 minutes. Figure 1 provides an example of performance of a test in a setting where the prevalence is low.

This is corroborated by a study that found serial screening using RADTs becomes less cost-effective as transmission rates drop.Provincial and territorial governments are well placed to weigh the cost of distributing free or inexpensive self-tests for public health purposes.Businesses and private enterprise are also well placed to weigh the cost of implementing their own self-test programs. The Government of Canada and some provinces have been working with industry associations, non-profits and other organizations to provide access to rapid testing in many sectors.Recommendations for self-testingThe online pharmacy ventolin Panelâs self-testing recommendations are based on the evidence available when this report was written.
The goal of the recommendations is to provide accessible testing and screening in order to identify positive cases, reduce community transmission of asthma treatment and facilitate re-opening in Canada. As additional data and evidence become available, the Panel may need to revisit these recommendations.CommunicationRecommendation 1 Self-testing means that an individual is responsible for independently performing the entire testing process. For this reason, self-tests online pharmacy ventolin should come with clear, concise messaging.
